The situation
A healthcare admissions office kept its forms, records and PDFs across many Google Sheets. Staff chased files from sheet to sheet to fill one patient folder.
What we built
A web portal with 18 structured forms, add and edit, a two-page PDF output, signature areas, staff and admin roles, and two-step login. Every submitted form is saved to the person it belongs to.
