Care office: 18 paper forms in one portal, same printout
A care admissions office ran on 18 paper forms and a stack of spreadsheets. Now staff pick a patient, the forms fill from one profile, and the PDF prints in the layout they already know.

Before and after

What we built
- 18 forms, one profile. Pick a patient and every form fills from the same record.
- The same printout. A two-page PDF with signature areas, laid out like the paper form.
- Medications required. The field can never be left blank.
- Each person sees their part. Separate staff and admin logins, with a second step at sign-in.
- Yours to keep. Works on phones. Code and database handed to the owner.
